To register for a Vasectomy, follow the steps below:

  1. Review all the Patient Information on this site.
  2. Complete the Patient Registration Form
  3. Complete the Elective Sterilization Consult Form
  4. Complete the Appointment Request Form

The online registration procedure should take about 15 minutes to complete.
Please make sure that you fully complete the three forms listed above.
If you stop before completing all three forms, your data may not be sent correctly to our clinic.

In order to perform a vasectomy, our clinic will review your registration information before scheduling a consultation. The consultation is required to ensure that a vasectomy can be performed and is included in the vasectomy fee; during the consultation, Dr. Attia will discuss any initial concerns with you and, preferably, your partner, and will perform a physical examination to ascertain your general health and that a vasectomy can be performed in the office. Dr. Attia will also counsel you and your partner regarding the vasectomy procedure and possible complications and will give you pre-procedural instructions as applicable. Dr. Attia requires proof that you are both hepatitis negative and HIV negative, and if necessary, a blood sample may be drawn in our office for such verification.

The vasectomy procedure occurs at a later date. Dr. Attia prefers to use a minimally invasive procedure akin to a Non-Scalpel Vasectomy and will use alternative methods upon request. After the vasectomy, you are requested to come in should any problems arise related to your procedure. A successful vasectomy is confirmed through periodical semen analysis; you will be declared sterile after two consecutively negative semen analyses.

To register for a Vasectomy Reversal, follow the steps below:

In order to perform a vasectomy reversal, our clinic will review your registration information before scheduling a consultation. The consultation is required to ensure that a vasectomy reversal can be performed and is included in the vasectomy reversal fee; during the consultation, Dr. Attia will discuss any initial concerns with you and will perform a physical examination to ascertain your general health and the viability of the vas reversal procedure. Dr. Attia will also counsel you regarding the procedure and possible complications and will give you pre-procedural instructions as applicable. Dr. Attia may also take a blood sample, if needed, to evaluate testicular health.

The procedure occurs at a later date. Dr. Attia performs microsurgery vas reversal under general anesthesia. Follow up visits are recommended at six weeks and three months after the procedure. Should you have any problems before your six-week follow-up, please call or visit our office with your concerns.
